Job summary

Healthcare Alternative Systems (HAS) is seeking a Case Manager to oversee a caseload of justice-involved participants and connect them to housing, employment, medical, and legal resources. You will develop person-centered plans, monitor clinical engagement, and document progress to support wraparound reentry efforts.

The role requires a Bachelor's degree in a related field with at least two years of experience; CADC is preferred. Bilingual English/Spanish is highly valued.

Qualifications

• Bachelor's degree in Human or Social Sciences or Social Work required.
• Minimum two years of experience in a relevant field.
• CADC certification is preferred but not required.

Responsibilities

• Manage a caseload of justice-involved participants.
• Link participants to community resources and wraparound services.
• Develop individualized service and treatment plans with goals.
• Document all services in SAMMS and trackers.
• Collaborate with the treatment team and attend meetings.
