Job summary

ACMH is seeking a Senior Case Manager/Case Manager for Sabra Goldman House in Queens Village, NY to provide case coordination and deliver community rehabilitation services.

Reports to the Program Director. Schedule: Tue-Sat or Sun-Thu, 11:00 am–7:00 pm. Responsibilities include home visits, psychosocial histories, assessments, coordinating care, medication management, and developing individualized Service/Support Plans.

Qualifications

• MA or BA in psychology or a human services related field with a minimum of 3 years of experience providing direct care in human services.
• Experience working with adults with serious mental illness preferred.
• Strong written and verbal communication skills.

Responsibilities

• Conduct regular home visits for assigned caseload.
• Obtain psychosocial histories and conduct functional skills assessments.
• Establish and maintain liaison with primary psychiatric care providers.
• Ensure access to psychiatric, substance use, medical services and entitlements.
• Monitor client medication and side-effects.
• Administer standardized health screening tools.
• Develop an individualized rehabilitation plan (Service/Support Plan).
• Deliver community rehabilitation services including skills training and housing support.
• Document client response and update Service/Support Plan.
• Provide crisis intervention and staff emergency coverage as needed.
