Job summary

Texas Health and Human Services Commission (HHSC) is seeking a Case Manager IV at Austin State Hospital to lead advanced case management, collaborate with patients, families, and LMHAs, and drive discharge planning. The role emphasizes recovery planning, advocacy, and coordinated care across multiple providers.

The ideal candidate will have a degree in a related field (or equivalent experience), strong knowledge of social services, and the ability to work with diverse teams in a complex hospital

Qualifications

• Bachelor’s degree and a minimum of six months case management or social services work experience required.
• Associate degree and a minimum of twenty-four months experience case management or social services work required.
• No degree and a minimum of forty-eight months experience in case management or social services work required.

Responsibilities

• Oversee development of individualized goals and recovery plans; ensure patient and family involvement.
• Advocate for patient rights and provide information on rights and state/federal regulations.
• Coordinate discharge planning and community referrals to support continuity of care.
• Mentor junior case managers and participate in quality improvement activities.
