Job summary

Health & Human Services Commission in Austin is seeking a Case Manager IV to join the MultiDisablPsych Social Svs team at Austin State Hospital. This full-time role involves comprehensive case management for individuals, coordinating recovery plans and discharge planning.

The position requires a bachelor's or equivalent in social services with experience; or an associate with years of CM experience; or substantial experience without a degree.

Qualifications

• Bachelor’s degree and six months case management or social services experience required.
• Associate degree with twenty-four months of case management or social services experience required.
• No degree with forty-eight months of case management or social services experience required.

Responsibilities

• Attends work regularly and may rotate shifts, overtime as needed.
• Collects social assessment information from patients, family, and LARs.
• Oversees recovery plan goals and measurable objectives with patient involvement.
• Advocates for patient rights and provides information on rights and privacy.
• Coordinates discharge planning and informs families of changes.
