• Coordinate furloughs, transfers, and discharges with patient-centered planning.
• Design and deliver individual and group trainings to promote rights and transition to community options.
• Participate in interdisciplinary team meetings and assist with discharge planning.
• May supervise Social Worker I/II and students.
