Job summary

Texas Health and Human Services Commission (HHSC) seeks a Case Manager IV for Big Spring State Hospital. The role includes advanced case management, coordinating assessments, developing recovery plans, and advocating for patient rights across the care continuum.

Work with families, LMHAs, and the recovery team to ensure smooth discharge and continuity of care. Applicants must meet education criteria and have extensive case management experience.

Qualifications

• Bachelor’s degree and a minimum of six months case management or social services work experience required.
• Associate degree and a minimum of twenty-four months experience case management or social services work required.
• No degree and a minimum of forty-eight months experience in case management or social services work required.
• AND Four (4) years of experience in case management or social services work

Responsibilities

• Interact with patients, family, and LAR to assess needs.
• Develop individualized recovery plans with measurable goals.
• Advocate for patient rights and provide information on rights.
• Coordinate discharge planning and community resources.
• Support family involvement in recovery activities.
• Assist in quality improvement and utilization management activities.
