Job summary

Texas Health and Human Services Commission (HHSC) is seeking a Social Worker III at the Austin State Hospital to provide highly specialized social services, coordinate care, and advocate for patients within a multidisciplinary team.

The role focuses on complex cases, discharge planning, community placement coordination, and adherence to state hospital policies. Eligible candidates include LMSW/LCSW with relevant experience in mental health services.

Qualifications

• Licensed LMSW or LCSW in Texas.
• Minimum one year experience with individuals with mental, social, or substance use disorders.
• Must pass background checks, drug screen and related pre-employment checks.

Responsibilities

• Provide complex social services, assess support systems and coordinate care.
• Serve as liaison with patient, representatives, and care teams to ensure transitions.
• Design and deliver trainings to promote rights and person-centered planning.
• Oversee discharge planning, furloughs, transfers, and community placements.
• Participate in interdisciplinary team meetings and advocate for patient needs.
• Supervise and mentor Social Worker I/II and students as needed.
