Dungarvin inc seeks a Senior Case Manager in Minnesota to help individuals live in the community by coordinating supports and services. The role focuses on assessment, planning and advocacy, empowering clients to make their own decisions with guidance from the case management team.

The position emphasizes education and resource provision for clients and peers, contacting a wide range of organizations, families and community members with professional conduct.

Qualifications

• Bachelor’s Degree in a human service-related field and two years of experience working with individuals with disabilities or aging.
• Two years fulltime experience in CADI/DD waiver case management.
• Knowledge of the local community; good oral and written communication skills.
• Knowledge of self-directed services concepts.
• Valid driver’s license and proof of automobile insurance.

Responsibilities

• Coordinate services for vulnerable individuals and help them navigate the service delivery system.
• Employ a systematic process of ongoing assessment, planning, referral, service coordination, monitoring and advocacy.
• Directly coordinate a caseload and facilitate/coordinate other services to positively influence individuals’ lives.
• Act professionally and represent the interests of the individuals and the department.
