Job summary

Dungarvin is seeking a dedicated staff member to provide direct support for individuals with intensive behavioral and mental-health needs in our group homes in Minnesota. You will implement person-centered strategies, assist with daily living activities, and document care using electronic systems such as T-logs and Therap.

Responsibilities include following behavior support plans, de-escalation techniques, crisis response, and coordination with supervisors and care teams.

Qualifications

• High school diploma or GED required.
• 3 years minimum of experience with individuals with chronic and severe mental illness OR a degree in a mental-health-related field.
• Must be at least 18 years old; some programs require 21+.
• Valid driver5s license and proof of auto insurance.
• Basic computer skills and ability to use electronic documentation systems.

Responsibilities

• Provide direct support to individuals with intensive behavioral and mental-health needs, using person-centered approaches.Support individuals in accordance with behavior support plans and crisis-prevention strategies.Maintain a clean, safe living environment; assist with daily living skills, tasks, and transportation.Document services, observations, health concerns, and activities using electronic systems (T-logs, Therap, health tracking).Monitor behavioral changes and communicate with supervisors during crises or safety situations.Support family contacts, assist with appointments, and follow medical/nutrition plans.
