Job summary

DUNGARVIN is seeking a Senior Case Manager to coordinate supports for individuals living in the community, helping them make informed choices and access services. The role focuses on assessment, planning, referral, service coordination, monitoring and advocacy to improve clients’ lives.

The position requires a Bachelor’s in a human-service field and CADI/DD waiver case management experience, with a strong emphasis on self-direction and community knowledge. Competitive salary and benefits offered.

Qualifications

• Bachelor’s degree in a human service-related field and two years of experience working with individuals with disabilities or aging
• Two years full-time experience in CADI/DD waiver case management
• Knowledge of the local community
• Good oral and written communication skills
• Knowledge of the concepts related to self-directed services
• A valid driver’s license
• Proof of automobile insurance

Responsibilities

• Coordinate services for vulnerable individuals and assist them to navigate the complex service delivery system.
• Directly coordinating a caseload and facilitating/ coordinating other services or supports.
• Represent the interests of the individuals and of the case management department in a professional manner.
