Job summary

Institute for Community Living Inc. (ICL) is seeking a dedicated Case Manager to support consumers living in independent apartments within the community.

You will assist with medical, social, psychosocial, educational, and vocational services to promote independent living and crisis intervention when needed. You will conduct assessments, develop and monitor treatment plans, and maintain confidential case records while coordinating with care teams and outside resources.

Qualifications

• Bachelor’s degree required with ~1.5 years of direct service experience.
• Experience working with individuals with mental disabilities preferred.
• Ability to work independently and as part of a team while following regulations.

Responsibilities

• Travel to consumers' residences to provide supportive services.
• Develop and maintain consumer treatment plans and quarterly reviews.
• Maintain accurate case records and progress notes for each consumer.
