Job summary

Institute for Community Living Inc. seeks a Senior Case Manager to join a team supporting adults in community living arrangements.

You will coordinate services, develop treatment plans, monitor progress and document case records to ensure quality care and regulatory compliance. Responsibilities include crisis intervention, client advocacy, coordinating with medical, social and vocational resources, and providing guidance to junior staff.

Qualifications

• Bachelor’s degree plus 1.5 years’ experience providing direct services to individuals with mental disabilities.
• Minimum experience requirements are waived for candidates with a bachelor’s degree that requires a one-year practicum working directly with individuals with mental disabilities (e.g., BSW); or an associate's degree in a human service-related field and two years of experience in human services.

Responsibilities

• Travel to consumers' residences to provide supportive services on a schedule set by the program.
• Develop and maintain consumer treatment plans with quarterly reviews and updates.
• Maintain up-to-date case records and document progress in the system.
• Provide 1:1 or group counseling to modify attitudes and behaviors as needed.
• Refer consumers to community resources and accompany to medical/social appointments.
• Coordinate with team members to meet discharge planning goals.
• Review service plans and ensure cultural/linguistic considerations are included.
• Assist with program audits and training of staff as needed.
