Job summary

Arizona Department of Economic Security (DES) seeks a Case Manager 3 to join the Division of Developmental Disabilities. The role focuses on developing Person-Centered Service Plans, coordinating supports, and guiding families through community resources.

The position requires RN licenses or a relevant bachelor’s degree with case management experience, strong HIPAA knowledge, and the ability to lead teammates. Extensive driving and background checks are part of the job.

Qualifications

• Arizona RN license in good standing OR Bachelor’s in Social Work, Psychology, Special Education, or Counseling with at least 1 year of case management experience.
• Bachelor’s Degree in Human Services or related field with 1 year of case management experience OR 3 years’ experience with individuals with intellectual or developmental disabilities.

Responsibilities

• Drive a vehicle to conduct onsite visits to members’ homes, schools, or program sites.
• Develop, review, and monitor Person-Centered Service Plans (PCSP) and refer to community services.
• Facilitate meetings and program visits to assist member/family choices and complete referrals.
• Collaborate with community agencies to assist members and families in crisis.
• Write progress notes, maintain case records, and prepare reports; participate in staffing meetings and training.
