Job summary

The Arizona Department of Economic Security (DES) seeks a Case Manager 3 in Chandler, AZ to support individuals with developmental disabilities and their families. You will visit members in their homes, develop Individual Support Plans, and coordinate with multiple agencies to ensure appropriate services and referrals.

Responsibilities include documenting progress, attending staff and statewide meetings, and guiding a team of coordinators in the supervisor's absence.

Qualifications

• Knowledge of developing Individual Support Plans and related procedures.
• Knowledge of federal/state statutes and agency policies.
• Understanding of case management principles and community resources.
• Ability to write clear progress notes and maintain case records.

Responsibilities

• Drive to on-site visits to members/families and develop relationships; convene ISP meetings.
• Provide crisis support and attend meetings with members/families and providers.
• Gather data to determine eligibility determinations and re-determinations.
• Prepare reports and maintain case records; attend trainings and meetings.
