Job summary

The Department of Economic Security (DES) in Arizona is seeking a Case Manager 3 in Surprise, AZ. The role supports individuals with developmental disabilities and their families, coordinating services and documenting progress.

Des provides flexible, high-quality services that empower choice and independence. The successful candidate will participate in team-based care, maintain confidentiality, and drive initiatives within the Division of Developmental Disabilities.

Qualifications

• Arizona licensed RN OR Bachelor in Social Work OR Bachelor in Psychology/Special Education/Counseling with 1+ year case management experience for elderly and/or developmental disabilities or SMI.
• Bachelor’s degree in Human Service or related field with 1+ year case management experience for elderly and/or developmental disabilities and/or SMI.
• Arizona Level 1 Clearance Card is required.
• Background checks and E-Verify are required.

Responsibilities

• Drive to members'/families' homes, schools, or program sites to build relationships and discuss needs.
• Develop, write, monitor, and evaluate ISP with timely documentation.
• Research community resources to meet ISP needs and inform members of options.
• Facilitate meetings and referrals for services.
• Collaborate with community agencies to provide support to members in crisis.
• Mentor and train new and current team members.
