Job summary

Comunilife is seeking a dedicated Case Manager for its congregate program in the Bronx. You will provide comprehensive case management, develop integrated recovery plans, and coordinate services to assist clients and families.

The role requires a bachelor’s or related degree with relevant experience, plus the ability to conduct monthly home visits and coordinate with healthcare and social services. In-office position with some field work, strong communication, and knowledge of housing guidelines

Qualifications

• Bachelor’s Degree in human services or related field with two (2) years’ case management experience
• Associates Degree in human services or related field with four (4) years’ case management experience
• High School Diploma with six (6) years’ case management experience
• Credentialed Alcohol and Substance Abuse Counseling (CASAC-T or CASAC) a plus
• Knowledge of HRA Congregate Housing Program guidelines
• Direct service experience with persons living with HIV/AIDS, substance abuse, serious mental illness, and homelessness
• Bilingual English/Spanish a plus
• 5 Day/35 hour work week Non-Remote Position (In-Office/Some Field Work)
• Excellent oral and written communication skills
• Proficient in Google Workspace/Microsoft Office Suite
• Familiar with AWARDS, CAIRS, CAPS

Responsibilities

• Ensure that each client receives appropriate case management services
• Conduct comprehensive assessments and develop integrated recovery plans
• Conduct home visits and office visits with regular frequency
• Assist with access to primary medical care, mental health, and substance abuse services
• Provide crisis intervention to clients and families as needed
• Maintain documentation of contacts and services provided
• Coordinate supportive services and referrals
• Collaborate with service providers and vocational supports
• Participate in supervision and trainings
