Job summary

Catholic Charities of the Archdiocese of Washington seeks a Lead Case Manager to improve the well-being of individuals and families experiencing homelessness through coordinated case management and community resources. You will ensure program operations adhere to best practices, contribute to program development and staff training, and support quality assurance via data review in HMIS and related systems.

Qualifications

• Master’s degree in Social Work with active LGSW or LPC licensure.
• Bachelor’s in Human Services or Psychology with four years of PSH experience.
• Two years’ experience with homeless individuals or families with mental health/substance issues.

Responsibilities

• Ensure contracts, policies, accreditation, ethics and quality standards are followed.
• Monitor client files in HMIS/HTH-Octo for data completeness and quality.
• Ensure notes use DAP format and are submitted within 48 business hours.
• Lead client file reviews and report findings to program leadership.
• Facilitate client satisfaction surveys and staff supervision.
• Coordinate case conferences and staff training on best practices.
• Review incident reports and liaise with DHS and QA staff as needed.
• Oversee intake referrals and assign cases to Case Management staff.
