Job summary

Catholic Charities of the Archdiocese of Washington is seeking a Case Manager to deliver direct case management and coordinate community resources for clients in need.

The role requires a Bachelor's degree in human services, licensure in social work or professional counseling where applicable, and at least one year of experience with at-risk populations. A valid driver's license is preferred; schedule is Monday–Friday, 9:00 AM–5:30 PM.

Qualifications

• Bachelor's degree required in human services or related field.
• Social work licensure required where applicable.
• Professional counseling licensure required where applicable.
• Minimum of one year experience with at-risk clients in direct service.

Responsibilities

• Interview clients to assess direct service needs and develop case plans with goals.
• Make referrals and reassess goals with clients at regular intervals.
• Lead individual and group meetings and collaborate with a multidisciplinary team.
• Coordinate with substance abuse, legal, medical, educational and housing service providers.
• Maintain case records, documentation, and program data to standards.
