Job summary

Catholic Charities of the Archdiocese of Washington is seeking a Lead Case Manager to improve the well-being of current or formerly homeless individuals and families through case management and coordination of community resources.

The role supports program operations, staff training, and program development while ensuring compliance with contract/grant requirements and agency practices. Monday–Friday schedule.

Qualifications

  • Master’s degree in Social Work with LGSW or LPC.
  • Bachelor’s degree in Human Services or Psychology with four years of work experience.
  • Two years of experience with homeless or mental health/substance abuse populations.

Responsibilities

  • Work with Manager of Social Work to ensure contract compliance and quality standards.
  • Monitor client files in HMIS and HMIS-related systems for completeness and data quality.
  • Lead client file reviews and report findings to leadership.
  • Facilitate client satisfaction surveys and staff training sessions.
  • Coordinate intake referrals and assign cases to Case Management staff.
  • Provide crisis intervention and coordinate services for families and individuals.
  • Complete program reports and maintain statistical data to meet standards.

Job Summary

The Lead Case Manager improves the well-being of current or formerly homeless individuals and families by providing guidance and case management services, including the coordination of community resources. The position helps ensure that operations and practices follow best practice standards of the profession, contract/grant requirements, and agency case management practices and procedures. The Lead Case Manager provides input on the program operations, program development, staff training and some personnel activities.

Essential Duties and Responsibilities
  • Work with the Manager of Social Work to ensure compliance with contracts, agency policies and procedures, departmental guidelines, accreditation requirements, ethics, best practices, and quality standards within the field.
  • a. Monitor and review client files and client information in assigned databases such as Homeless Management Information System (HMIS) and HTH/Octo for completeness and quality of content.
  • b. Review the quality of notes and ensure they are in Data Assessment Plan (DAP) format and contain submission of case notes within 48 business hours of service provided.
  • c. Lead and participate in client File Reviews (the agency's Individual Service Review process); report any findings to the Manager of Social work and Director of Quality Assurance.
  • d. Facilitate random client satisfaction surveys for quality assurance and staff supervision.
  • e. Facilitate regularly scheduled case conference and skill-building meetings for case management staff to build competencies in best practices and department practice protocols.
  • f. Review incident reports, liaison with (Department of Human Services (DHS) and Department of Quality Assurance staff as needed.
  • g. Report all serious incidents, allegations, grievance and sensitive situations to the Sr. Program Manager and Director of PSH in a timely manner.
  • Oversee direct service operations to improve client services outcomes and customer satisfaction.
  • a. Serve as point of contact for all new program referrals at the time of intake and assign them to Case Management staff.
  • b. Report clients' vacancies to the funder and serve as a point of contact for the Coordinated Entry System (CAHP). In addition, attend all necessary CAHP meetings/conference calls
  • c. Provide crisis intervention assistance to all Case Management staff, in the absence of assigned case manager supervisor.
  • d. Deliver and coordinate services to individuals and families, as needed.
  • e. Completes reports and maintains statistical data, meets programmatic, documentation and other performance standards, on an on-going basis.
  • f. May carry a limited caseload, depending on the needs of the program.
  • May supervise volunteers and/or interns.
  • Perform other job-related duties as assigned.
Requirements
Education and Experience
  • Master's degree in Social Work. Must have an LGSW, or LPC.
  • Bachelor's Degree in the Human Services or Psychology field with four (4) years of work experience providing Permanent Supportive Services to Individuals/families.
  • Two years of experience working with individuals and/ or families who are homeless or have mental health or substance abuse issues.
Skills and Competencies
  • Driving is required. Must have a valid driver's license and clean driving record. Driving in the community to clients' homes is expected occasionally.
  • Skill in the use of computers, preferably in a PC, Windows-based operating environment, especially Office 365 applications.
