Job summary

Department of Economic Security (DES) in Arizona is seeking a Case Manager 3 to support adults with developmental disabilities. You will manage PCSPs, drive to client sites, and coordinate services with families and agencies to promote independence and safety.

Qualified candidates include Arizona-licensed RNs or BSW/SW degree holders with relevant case management experience. A valid driver's license and clear background are required; team leadership possible in supervisory capacity.

Qualifications

• Bachelor's Degree in Social Work, Human Services, Psychology or Counseling or RN license with relevant case management experience.
• Minimum 1 year of case management for elderly/disabled or SMI; 2+ years for some tracks.
• Driver’s license valid for state business; vehicle use required.

Responsibilities

• Drive to members' homes, schools, and program sites to build relationships and discuss needs.
• Develop and monitor Person-Centered Service Plans (PCSP) with timely updates.
• Facilitate meetings and referrals for services; document progress and referrals.
• Collaborate with community agencies to assist members and families in crisis.
• Prepare reports, maintain case records, and attend meetings; may supervise in absence of supervisor.
