Job summary

The Department of Economic Security (DES) Division of Developmental Disabilities in Chandler, AZ is seeking a Case Manager 2 to support individuals with developmental disabilities. Responsibilities include PCSP development, coordinated service referral, and collaboration with providers to promote community engagement.

Qualifications include a Bachelor’s degree in a related field and experience in case management; bilingual Spanish/English is preferred.

Qualifications

• Bachelor's degree in a human service field or related area.
• Experience providing case management to elderly or disabled individuals preferred.
• Fingerprint clearance card required prior to employment.

Responsibilities

• Drive to members' homes, schools, and program sites to assess service needs.
• Develop, review, and monitor Person-Centered Service Plans (PCSP) with timely progress notes.
• Facilitate meetings and referrals for services and supports.
• Collaborate with community agencies to assist members and families in crisis.
• Write progress notes and prepare reports and special projects.
