Job summary

Arizona State Government is seeking a seasoned Case Manager 3 in Benson, AZ to support the Division of Developmental Disabilities. You will drive to member homes, develop PCSPs, and coordinate services with families and agencies to improve quality of life.

The role requires a relevant bachelor’s degree (or RN) with 1+ year of case management experience and ability to work in a collaborative team. Driver’s license, fingerprint clearance, background checks, and E-Verify are required.

Qualifications

• Bachelor’s Degree in Social Work, Psychology, Special Education, Counseling, or related field; or RN with relevant experience.
• 1+ year of case management experience for elderly or disabled or SMI individuals; or 3+ years for other pathways.
• Valid driver’s license and, if required, driver’s record checks.
• Fingerprint clearance card; background checks; E-Verify.

Responsibilities

• Drive to members' homes and sites to assess needs and develop relationships.
• Develop, review, and monitor Person-Centered Service Plans (PCSP) with timely documentation.
• Lead or assist with meetings and referrals for services.
• Collaborate with community agencies to support members and families in crisis.
• Write progress notes and maintain case records; prepare reports.
