Job summary

Department of Economic Security (DES) in Arizona is seeking a Case Manager 3 to join the Division of Developmental Disabilities. The role focuses on person-centered planning, service coordination, and collaboration with families, schools, and community agencies.

The position requires a Bachelor’s degree in human services or related fields, plus relevant case management experience; RN licensure in Arizona is acceptable or equivalent qualifications.

Qualifications

• Experience developing and monitoring person-centered service plans (PCSP).
• Ability to interview, counsel, and work with families and members.
• Strong understanding of HIPAA and community resources.
• Knowledge of policies related to developmental disabilities and case management.

Responsibilities

• Drive to members’ homes, schools, or sites to assess service needs.
• Develop, review, and monitor PCSP with timely updates and referrals.
• Facilitate meetings and coordinate services for members and families.
• Collaborate with community agencies to provide support during crises.
• Maintain case records and prepare required reports and documents.
