Qualifications

• Bachelor's Degree in related field and 1 year of related experience or 2 years of related experience.
• Must be able to communicate effectively in writing.
• Strongly prefer understanding of and competence in serving culturally diverse populations.
• Must be able to read English and communicate effectively in the primary language of the program.

Responsibilities

• Collaborate with the Team Leader to provide continuous quality improvement of individualized programs and services.
• Assist team members with the development of program curriculum to promote individual growth and self determination.
• Develop and implement comprehensive written therapeutic service plans/individual support plans for all assigned consumers.
• Convey changes in consumer goals to the larger internal support team.
• Maintain member records including timely completion of assessments, progress reports and documentation.
• Compile review summaries for therapeutic service plans/individual support plans.
