Job summary

SilverStay is seeking an experienced Case Manager (RN/LMSW/LCSW) to manage complex hospital discharges for partner facilities. The role requires on-site availability and a track record in inpatient discharge planning and post-acute navigation.

You will collaborate with hospital teams and care networks to ensure safe, timely transitions and communicate progress to leadership. You will leverage data-driven approaches to improve LOS, readmission rates, and discharge success, while maintaining

Qualifications

• Must be a licensed RN, LMSW, or LCSW.
• 3+ years on-site hospital, adult in-patient case management, discharge planning, or transitional care experience.

Responsibilities

• Facilitate safe and successful discharges for complex patients across hospital and post-acute settings.
• Navigate barriers and drive innovative discharge solutions within SilverStay’s network.
• Participate in rounds, huddles, and communicate with patients and families for seamless coordination.
• Present reports to hospital leadership and refine scalable workflows to improve LOS and outcomes.
• Track metrics (LOS, readmission, discharge success) and drive continuous improvement.
