Job summary

SilverStay is seeking an experienced Case Manager to oversee complex patient discharges at a partner hospital in the Towson, MD area. The role requires on-site hospital experience with SNFs, LTACHs, and rehab settings, focusing on efficient, patient-centered discharge planning.

You will collaborate with care teams, families, and leadership to reduce LOS, improve outcomes, and scale innovative workflows across care transitions. BLS certification is required.

Qualifications

• Must be a licensed RN, LMSW, or LCSW
• Must have 3+ years' on-site hospital, adult in-patient case management, discharge planning, or transitional care experience.
• Skilled at navigating post-acute settings (SNF, LTACH, acute rehab, home health).
• A patient and family-centered communicator with empathy, professionalism, and persistence.
• Ambitious, entrepreneurial driven, innovative, dedicated and diligent work ethic.
• Thrives in high-responsibility, high-ambiguity environments where adaptability and creativity are key.
• Tech-savvy and excited about the role of innovation in healthcare.
• Must be BLS certified.

Responsibilities

• Facilitate safe and successful discharges for complex patients by navigating barriers and driving innovative solutions across hospital teams and post-acute care settings within SilverStay’s preferred provider network, including SNFs, LTACHs, acute rehab, home health, and community resources.
• Use creativity and resourcefulness to solve discharge challenges without the constraints of hospital bureaucracy.
• Participate in hospital team rounds, care team huddles, and engage with patients and families to ensure seamless communication across all stakeholders.
• Present reports to hospital leadership on progress, scalability efforts, and results, proposing and executing adjustments as needed.
• Build and refine scalable workflows and processes that increase efficiency and outcomes.
• Partner with SilverStay leadership and hospital teams to track key metrics (LOS, readmission rates, discharge success) and continuously improve results.
• Leverag(e) technology and data-driven insights to transform how complex cases are managed.
