Job summary

SilverStay is seeking experienced Case Managers (LCSWs, MSWs, and RNs) to manage complex hospital discharges. You will collaborate with hospital teams and post-acute providers to coordinate seamless transitions, navigate barriers, and drive high-quality discharge plans for complex patients.

This role offers autonomy, flexible thinking, and a fast-paced environment, with a salary range of $90k–$130k plus performance bonuses.

Qualifications

• Must be a licensed RN, LMSW, or LCSW.
• 3+ years of on-site hospital inpatient case management or discharge planning experience.
• Experience navigating post-acute settings (SNF, LTACH, rehab, ALF).
• Experience using EMR referral platforms such as Telligen, Careport/WellSky, EPIC, and Cerner.
• Patient- and family-centered communicator with empathy and professionalism.
• Entrepreneurial mindset with ability to own projects and innovate.

Responsibilities

We are seeking Case Managers (LCSWs, MSWs, & RNs) to manage complex discharges. Unlike traditional hospital case management, this role gives you the chance to innovate, collaborate, and directly impact patients and families during critical transitions of care.

What You’ll Do

• Facilitate safe and successful discharges for complex patients by navigating barriers and driving innovative solutions across hospital teams and post-acute care settings within SilverStay’s preferred provider network — including SNFs, LTACHs, rehab, home health, and community resources.
• Use creativity and resourcefulness to solve discharge challenges without the constraints of hospital bureaucracy.
• Participate in team rounds, care team huddles, and engage with patients and families to ensure seamless communication across all stakeholders.
• Present reports to hospital leadership on progress, scalability efforts, and results, proposing adjustments as needed.
• Build and refine scalable workflows and processes that increase efficiency and outcomes. Partner with SilverStay leadership and hospital teams to track key metrics (LOS, readmission rates, discharge success) and continuously improve results.
• Leverage technology and data-driven insights to transform how complex cases are managed.

Compensation & Benefits

• Competitive Salary: $90,000 - $130,000; commensurate with experience and potential impact. ? Performance-Bonus: Bonus structure that can add 10% to 40% of base salary, tied directly to outcomes and results. Paid out quarterly.

Requirements

• Must be a licensed RN, LMSW, or LCSW.
• Must have 3+ years’ on-site hospital, adult inpatient case management, discharge planning, or transitional care experience. This is a critical requirement.
• Skilled at navigating post-acute settings (SNF, LTACH, acute rehab, ALF, group home).
