Job summary

SilverStay is seeking experienced Case Managers (LCSWs, MSWs, and RNs) to manage complex hospital discharges. You will collaborate with hospital teams and post-acute providers to coordinate seamless transitions, navigate barriers, and drive high-quality discharge plans for complex patients.

This role offers autonomy, flexible thinking, and a fast-paced environment, with a salary range of $90k–$130k plus performance bonuses.

Qualifications

• Must be a licensed RN, LMSW, or LCSW.
• 3+ years of on-site hospital inpatient case management or discharge planning experience.
• Experience navigating post-acute settings (SNF, LTACH, rehab, ALF).
• Experience using EMR referral platforms such as Telligen, Careport/WellSky, EPIC, and Cerner.
• Patient- and family-centered communicator with empathy and professionalism.
• Entrepreneurial mindset with ability to own projects and innovate.

Responsibilities

• Facilitate complex discharge planning across hospital and post-acute settings.
• Collaborate with care teams to remove discharge barriers and improve LOS metrics.
• Engage patients and families with clear, compassionate communication.
• Present progress and outcomes to hospital leadership; adjust as needed.
• Develop scalable workflows to improve efficiency and outcomes.
• Leverage data and technology to optimize complex case management.
