Job summary

SilverStay is seeking an experienced Case Manager (LCSWs, MSWs, & RNs) to manage complex patient discharges for partner hospitals in the Maryland area. The role emphasizes autonomy, innovation, and collaboration with hospital teams and post-acute providers to ensure safe, timely transitions.

You will navigate barriers, drive scalable workflows, and report outcomes to leadership, while maintaining patient- and family-centered communication.

Qualifications

• Must be a licensed RN, LMSW, or LCSW.
• Must have 3+ years' on-site hospital, adult in-patient case management, discharge planning, or transitional care experience.

Responsibilities

• Coordinate safe discharges for complex patients across hospital and post-acute settings.
• Identify barriers and drive innovative discharge solutions with care teams.
• Participate in rounds, huddles, and communicate with patients and families.
