Job summary

Institute for Community Living Inc. (ICL) is seeking a compassionate case manager to support consumers living in independent apartments across the community.

You will help individuals develop living, working, and social skills while coordinating medical, psychosocial, educational, financial, and vocational services and providing crisis intervention as needed. You will travel to consumer residences, maintain case records, and collaborate with the team to ensure recovery-oriented, person-centered

Qualifications

• Bachelor's degree plus two years’ experience providing direct services to individuals with mental disabilities or related human service experience.
• Experience using case management processes and collaborative care planning.
• Familiarity with community resources and service coordination.

Responsibilities

• Travel to consumers' residences to provide supportive services and assist with service planning.
• Maintain up-to-date case records, service plans, and progress notes.
• Provide crisis intervention and connect individuals to medical, social, psychosocial, educational, financial, and vocational services.
• Advocate for consumer access to community resources and coordinate with the team.
