Qualifications

• Bachelor’s degree in a human services field
• Associate’s degree in a human services field with two years of direct service experience
• High school diploma/GED with four years of direct human services experience

Responsibilities

• Conduct home visits to provide supportive counseling and independent living assistance.
• Assist consumers in accessing community resources, benefits, and vocational opportunities.
• Coordinate services with care managers, ICL staff, and external providers.
• Perform crisis assessment and intervention; refer to appropriate services.
• Advocate for consumer needs within ICL and with external providers.
• Maintain accurate case records, documentation, assessments, and quarterly reviews.
