Job summary

Kindred Hospital Paramount and Acute Rehabilitation Unit in Paramount, CA is seeking a Registered Nurse Case Manager to coordinate patient care across the continuum. Qualifications

• RN licensure required per state regulations.

Responsibilities

• Coordinate clinical and psychosocial activities with the Interdisciplinary Team and Physicians.
• Monitor patient stays to ensure effective care coordination and resource use.
• Refer high-risk patients for additional supports as needed.
• Serve as patient advocate and support informed decisions for patients and families.
• Participate in rounds to review goals and optimize resource utilization.
• Plan and manage discharge planning for safe transitions.
• Conduct medical necessity reviews from admission through discharge.
