Job summary

Kaiser Permanente in California is seeking a Registered Nurse for case management to coordinate admissions, screen medical necessity for inpatient stays and develop cost-effective discharge plans.

You will collaborate with MDs, families and multidisciplinary teams to ensure safe transitions, advocate for patients, and coordinate post-acute care across facilities while complying with regulatory requirements.

Qualifications

• Two (2) years RN experience in an acute care setting or case management required.
• Completion of an accredited RN training program that allows graduates to take RN license exam.

Responsibilities

• Plans, develops, assesses and evaluates care provided to members.
• Collaborates with physicians, other health care team members and patient/family to develop, implement and document care plans.
• Recommends alternative levels of care and ensures compliance with federal, state and local requirements.
• Assess high risk patients in need of post-hospital care planning.
• Develops and coordinates discharge plans to meet identified patient needs.
• Communicates the plan to physicians, patient, family/caregivers, staff and community agencies.
• Reviews, monitors and coordinates the hospital stay to ensure timely and efficient services.
• Coordinates transfer of patients to appropriate facilities; maintains required documentation.
