Job summary

Kindred Hospital Paramount is seeking a care coordination professional to support patient flow across the continuum. This role collaborates with the Interdisciplinary Team to ensure efficient resource use, high-quality discharge planning, and appropriate utilization management.

The candidate will review medical necessity, coordinate with payors, and serve as a patient advocate, ensuring age-appropriate and cost-effective care delivery.

Qualifications

• Knowledge of government and non-government payor practices and regulations.
• Medicare benefits and insurance processes and contracts.
• Excellent communication and interpersonal skills.

Responsibilities

• Coordinate clinical and psycho-social activities with the Interdisciplinary Team and Physicians.
• Assist with care coordination and facilitation; advocate for patients.
• Conduct comprehensive discharge planning and timely planning.
• Perform medical necessity reviews and coordinate continued stay with payors.
• Participate in rounds, conferences, and cross-functional care plans.
