Job summary

You will oversee case notes, coordinate with consumers, families, support coordinators, and staff to implement outcomes identified in annual support plans. The role requires a Bachelor’s degree in social or human services (or equivalent paraprofessional experience), strong written and analytical skills, and the ability to supervise staff while ensuring quality

Qualifications

• Bachelor’s degree in Social Work, Sociology, Psychology, Special Education, or related field from four-year college or university; five years paraprofessional experience may substitute for the degree.
• Ability to read, analyze, and interpret professional journals, reports, or regulations; ability to write reports and related correspondence; ability to present information clearly.
• Strong written and analytical skills with ability to communicate with diverse stakeholders.

Responsibilities

• Directly supervises training specialist in adult training areas and handles supervisory responsibilities per policy.
• Coordinate case management activities, including collection, organization, and analysis of information for vocational and residential planning.
• Attend and chair meetings, document minutes, and provide feedback on training methods.
• Ensure Medicaid Waiver compliance and quality assurance for program documentation and data.
