Qualifications

• Bachelor's degree in a human services field and 3 years full-time or equivalent experience with adults with severe and persistent mental illness.
• Or Bachelor's degree and 5 years full-time or equivalent with adults with persistent mental health diagnosis.

Responsibilities

• Provide case management and life skills services to clients.
• Develop individualized service plans and coordinate housing and services.
• Assist clients with daily living activities, transportation, and access to community resources.
• Maintain client records and documentation in the EMR within 48 hours of service.
• Engage clients in community integration and outreach to housing programs.
• Ensure safety and compliance with agency policies and procedures.
