Job summary

Apalachee Center, Inc. is a North Florida behavioral health provider seeking a full-time Case Manager/Support Coordinator.

You will help clients access benefits, coordinate care with CSU and other services, and maintain thorough documentation of progress and treatment plans. Responsibilities include risk assessments, developing service plans with clients and families, and facilitating referrals to community resources.

Qualifications

• Bachelor's degree in a related human services field.

Responsibilities

• Assist clients in gaining access to financial, insurance, employment, medical and social services.
• Complete community support assessments and develop service plans with clients.
• Maintain high-quality documentation and progress notes in client records.
• Coordinate care between service providers and crisis stabilization services.
• Identify high-risk clients and conduct suicide risk assessments.
• Assist eligible clients with SOAR for SSI/SSDI benefits.
• Attend staff and unit meetings as required.
