Children's Home Society of Florida is seeking a Behavioral Health Case Manager to support children and families across homes, schools, and communities. The role emphasizes assessments, service planning, and resource coordination to drive meaningful outcomes.

Qualifications

• Bachelor's Degree Required in Human Services/Social Work or related degree.
• Minimum 1 year working with severely emotionally disturbed children (or 3 years if degree is outside of Human Services)
• Florida Driver's License with access to a reliable, insured vehicle
• Can work Mon-Fri, 8-5pm, with flexibility to work outside standard hours when needed
• Completion of AHCA-approved Targeted Case Management certification training within three months of hire

Responsabilités

• Conduct comprehensive assessments to identify client and family needs and develop individualized service plans that drive meaningful outcomes
• Connect children and families to critical community resources and treatment services
• Advocate for clients across schools, healthcare systems, behavioral health providers, and community organizations
• Provide in-home support and guidance to families
• Document progress and outcomes through our electronic health record system
• Participate in family service planning, case reviews, and community outreach efforts
