Job summary

Telecare Corporation in San Diego is seeking a Part-Time Case Manager III to provide primary support and case management for individuals with mental illness, helping them live as independently as possible in their community.

Under supervision, you will coordinate services, support treatment goals, and collaborate with a multidisciplinary team to ensure quality care, while complying with program policies and safety guidelines.

Qualifications

• Bachelor's degree in Social Services or Human Services and (1) year of experience or High School Graduation or G.E.D. equivalent and four (4) years of direct services in Mental Health or community services; an Associate's degree may be substituted for two (2) year of experience.
• Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
• Willingness to use the company vehicle to drive clients to appointments and groups, etc.

Responsibilities

• Serve as a primary support and case manager for individuals with mental illness and other co-occurring disorders, helping them live as independently as possible in their chosen community.
• Under direct supervision, performs job functions and meets position standards.
• Coordinate services and support treatment goals in collaboration with a multidisciplinary team.
