Qualifikationen

• Bachelor’s degree in social work or related field with one (1) year of social work or related experience; or any equivalent combination of training and experience.
• Must maintain a valid Ohio driver’s license. Must meet background check requirements.

• Assess client and/or caller needs and level of functioning to determine appropriateness of services.
• Develop care plan for service coordination based on care team recommendations.
• Provide information and referrals via telephone and face-to-face interaction with clients, families, caregivers, professionals, and the public.
• Assess financial eligibility for senior options and other community services; address billing discrepancies with providers.
• Prepare reports, case summaries, and documents for supervisory review; maintain confidential records.
