Job summary

CareStar, Inc. is seeking a Case Manager to support the Ohio Home Care program across Franklin, Delaware, Licking, Fairfield, Pickaway, Fayette, Madison, and Union Counties.

The role combines remote work with field visits to assess needs, develop care plans, and connect individuals with essential services. Responsibilities include eligibility evaluations, care coordination across health and community systems, and transition planning to ensure seamless services.

Qualifications

• Bachelor’s degree in Social Work, Counseling, Psychology, Nursing, or a related field.
• Licensed Social Worker (LSW), LISW, LPN or RN required.
• Minimum 1 year of home and community-based services experience in the last 5 years.
• Experience with individuals with chronic conditions or severe mental illness.
• Strong knowledge of Medicaid, behavioral health systems, and local community resources.

Responsibilities

• Conduct face-to-face eligibility evaluations and assessments (e.g., Adult Needs and Strengths Assessment).
• Must be available during business hours (8:00 a.m. – 5:00 p.m.) for duties including calls, home visits, and emails.
• Travel to and from the individual’s home, up to 2 hours each way.
• Develop and maintain person-centered care plans and Health and Safety Action Plans; reports to regulators; incident reports as needed.
• Coordinate and authorize services across behavioral health, medical, and community-based systems.
• Perform transition planning and care coordination during provider changes or settings.
