Job summary

CareStar, Inc. in Ohio seeks a Case Manager to support clients across Fairfield, Pickaway, Fayette, Madison, and Franklin Counties.

You will assess needs, build personalized care plans, and connect individuals with Medicaid and essential services while working remotely with periodic field visits. The role offers full-time employment with remote flexibility and regular daytime hours, a comprehensive benefits package, 401(k), PTO, holidays, and ESOP participation.

Qualifications

• Bachelor’s degree in Social Work, Counseling, Psychology, Nursing, or related field.
• Licensed Social Worker (LSW/LISW), Licensed Practical Nurse (LPN) or Registered Nurse (RN) required.
• Minimum 1 year of home and community-based services experience.
• Experience with Medicaid, behavioral health systems, and local resources.

Responsibilities

• Conduct face-to-face eligibility evaluations and assessments.
• Be available 8:00 a.m. – 5:00 p.m. for duties including calls, home visits, and emails.
• Travel to individuals’ homes up to 2 hours each way.
• Develop person-centered care plans and Health and Safety Action Plans; report to regulators.
• Coordinate services across behavioral health, medical, and community systems.
• Manage transition planning and provider changes.
• Assist with Medicaid access and essential services.
• Monitor service delivery for compliance with guidelines.
• Advocate and coordinate care for each participant.
• Follow CareStar policies and confidentiality requirements.
• Adhere to Acceptable Use Policy for CareStar systems.
