Union County, OH seeks a dedicated social services professional to perform home visits, assess client needs, and coordinate services for elders and their families. The role includes investigating Adult Protective Services cases, formulating care plans, and advocating for safe, effective support options.

Responsibilities also include documenting activities, testifying in court when required, and collaborating with community partners to ensure timely, quality services.

Qualifications

• Bachelor’s degree in a Human-Services related field or equivalent experience.
• Excellent verbal and written communication skills.
• Regular and predictable attendance.
• Provided professional customer service and acted in a positive, courteous manner.

Responsabilités

• Conducts home visits to assess client safety, cognition, coping skills, and environmental factors.
• Investigates APS cases with interviews and collateral contacts.
• Forms care plans from assessments to support elders in least restrictive settings.
• Completes referrals to law enforcement when needed and documents investigations.
• Performs mandated assessments and documents activities within required timeframes.
• Coordinates services with community partners and tracks progress in systems.
• Maintains client records and ensures quality service delivery to meet needs.
• Participates in court hearings and interdisciplinary team meetings as required.
• Educates clients about available services and advocates for their best interests.
• Maintains licensure and certifications applicable to the role.
