Job summary

Mount Carmel St. Ann's is seeking a Senior Coordinator, Case Management & Social Services to provide administrative and operational support for Case Managers and Social Workers in a fast-paced acute care hospital environment.

The role assists with discharge planning, post-acute service coordination, documentation, insurer referrals, and communication among patients, families, physicians, and community agencies. Strong organizational skills are essential.

Qualifications

• Associate's degree or high school diploma with relevant experience required; Bachelor's degree preferred.
• Medical assistant or LPN highly preferred.
• 5-7 years of customer service, Insurance knowledge, medical assistance or secretarial experience preferred; prior medical setting experience required.
• Ability to organize and utilize work hours effectively and with minimal supervision.
• Medical terminology is a must.
• Must be able to use Outlook, EPIC, Teams, Faxing, and phone knowledge.

Responsibilities

• Communicate information received from payers to utilization review nurse.
• Transmit continued stay reviews and track authorizations.
• Complete HENS/PASSR.
• Schedule PCP/follow up appointments.
• Fax and phone agencies to assist with discharge referrals and continuity of care.
• Assist with delivery of charity items and arrange transportation as needed for discharge patients.
• Complete PCS forms and compile packets for transport.
• Coordinate with facilities and care teams to ensure patient discharge planning and post-acute care.
