Job summary

Qualifications

• Must hold a current GA RN license and be a graduate of an accredited nursing program.
• BSN recommended to enhance case management practice.
• Minimum 2 years of nursing experience and at least 1 year in case management, utilization management, or discharge planning preferred, with CM certification preferred.

Responsibilities

• Collaborate with a health care team to evaluate options and services from admission to return to the community.
• Conduct initial and concurrent chart reviews for utilization review and identify patients needing CM follow-up.
• Perform biopsychosocial assessments and communicate with physicians regarding actions to reduce discharge delays.
