Qualifications

• GA RN license required.
• Graduate of an accredited school of nursing; BSN recommended.
• Minimum of 2 years' nursing experience and at least 1 year in case management, utilization management, or discharge planning preferred.
• Certification in case management preferred.
• Excellent communication, interpersonal, and critical thinking skills.

Responsibilities

• Perform interdisciplinary evaluation of patient needs from admission through return to the community.
• Conduct initial and concurrent chart reviews for utilization review.
• Identify patients requiring case management follow-up and coordinate discharge planning for timely post-hospital transitions.
• Communicate with external agencies to provide referrals and ensure smooth transitions.
• Assist in proper identification of principal and secondary diagnoses in collaboration with treating physicians.
