Job summary

The Christ Hospital Health Network is seeking an experienced Case Manager to oversee clinical review of acute bedded patients for medical necessity and to coordinate care across the continuum. You will facilitate appropriate stays, obtain payer authorizations, and lead discharge planning with patients, families, and care teams.

Responsibilities include monitoring level of care, liaising with physicians, and documenting all actions in Midas/EPIC.

Qualifications

• Bachelor’s Degree required or nursing licensure with BSN completion planned within 3 years.
• 3–5 years of medical/surgical nursing experience preferred.
• Experience in case management and utilization review is desirable.

Responsibilities

• Clinical review of acute bedded patients for appropriateness of admission and status.
• Monitor continued stay and level of care using approved criteria.
• Coordinate with external payers for authorization of services.
• Develop discharge plans and educate patients and families.
• Identify barriers to discharge and collaborate with multi-disciplinary teams.
• Document reviews in Midas/EPIC per UR process.
• Participate in hospital committees and data collection on quality and efficiency.
