Job summary

Trinity Health in Georgia is seeking a Case Manager to collaborate with an interdisciplinary team and manage a caseload from admission to community reentry. The role includes chart reviews for utilization review and discharge planning to optimize resource use.

The Case Manager will assess biopsychosocial factors, communicate with physicians and external agencies, and ensure accurate diagnosis documentation in collaboration with the treating physician. Day shift, full time.

Qualifications

• Current GA RN license.
• Graduate of an accredited school of nursing; BSN recommended.
• Minimum 2 years of nursing experience and 1 year in case management or discharge planning.
• Certification in case management preferred.
• Excellent communication, interpersonal and critical thinking skills.

Responsibilities

• Coordinate interdisciplinary evaluation of patient needs from admission to post-hospital transition.
• Conduct initial and concurrent chart reviews for utilization review and identify follow-up cases.
• Perform biopsychosocial assessments and communicate actions to reduce delays.
• Initiate discharge planning to ensure timely transition to post-hospital setting.
• Liaise with system and community resources and referral agencies.
• Ensure accurate identification of principal and secondary diagnoses with treating physicians.
