Job summary

Twin Cities Physicians is seeking a Next-Level Wound Care Nurse Practitioner to serve community-based senior care across assisted living and memory care settings in the Twin Cities. This role combines clinical leadership with hands-on wound management and collaboration with interdisciplinary teams.

Ideal candidates hold an active Minnesota CNP license (AGNP preferred) and can travel across the Twin Cities Metro area.

Qualifications

• Active Minnesota CNP license required; AGNP preferred.
• Experience in wound care, geriatrics, primary care, or post-acute settings preferred.
• Strong clinical judgment and ability to manage medically complex patients.
• Valid driver's license and ability to travel throughout Twin Cities Metro Area.

Responsibilities

• Provide advanced wound care assessments, treatment planning, and ongoing management.
• Conduct routine rounding visits across assisted living, memory care, and long-term care communities.
• Manage medications, diagnostics, treatment plans, and follow-up care.
• Collaborate with physicians, nurses, therapists, caregivers, and facility teams.
• Support chronic disease management and preventive care initiatives.
