Job summary

Kintegra Health in Shelby, NC seeks a Social Worker (MSW) to provide direct social work case management to Senior Total Life Care participants in a home- and community-based model. You will screen clients, coordinate services, and connect them with community resources, including home visits that may involve travel.

Requirements: Master’s in Social Work, 2 years health-related experience (1 year with frail elderly preferred), valid NC driver’s license and vehicle, and up-to-date immunizations.

Qualifications

• Master's Degree in Social Work from an accredited program.
• 2 years health-related experience preferred; at least 1 year with frail elderly.
• Valid driver's license and vehicle.
• Immunizations up to date and medically cleared before direct participant contact.
• Meet standardized competencies for Senior TLC position.

Responsibilities

• Participates in initial assessments, care planning, and periodic reassessments for participants.
• Maintains current written case management records.
• Provides individual and family counseling, develops and leads group counseling and activities.
• Assists and advocates for participants’ access to services; refers participants and families to community agencies.
• Consults with and advises staff on social, emotional, and cultural factors related to health care; coordinates with community resources.
• Assists with coordination of 24-hour care delivery.
• Documents participant changes in the medical record and communicates updates to the team.
• Updates care plans throughout reassessment period.
• Participates with the Interdisciplinary Team and supports unity within the team.
